Recomposition on a GLP-1, Explained

Body recomposition means losing fat while keeping or building muscle. On a GLP-1, some of the weight you lose is lean mass, but strength training, protein at every meal and a steady pace help make more of the loss fat.

Short answer: Body recomposition means losing fat while keeping or building muscle. S01 On a GLP-1, some of the weight you lose is lean mass, which includes muscle, and the share varies widely between studies. S05 Cleveland Clinic says the muscle loss comes from losing weight, not from the drug itself. S02 Strength training, protein at every meal and a steady pace help make more of the loss fat. S02 S03 The scale can't show recomposition, so watch your strength, how your clothes fit, or a body composition test instead. S01

What body recomposition means

Most weight-loss goals focus on a number on the scale. Recomposition shifts the focus to what that weight is made of. Cleveland Clinic describes it as losing fat and building muscle at the same time, and it's careful to add that there is no simple fix or silver bullet. S01

Two people at the same weight can look and feel very different depending on how much of that weight is muscle. That's why body mass index alone doesn't tell the whole story. Providers can measure body composition with tools such as a DEXA scan or bioelectrical impedance. S01

Cleveland Clinic's version of recomposition rests on a few basics: strength training, a moderate cut in calories while keeping protein higher, and enough food to fuel your workouts. S01 Age, sex, training history and current body composition all affect how quickly results show up. S01

Why muscle comes up with GLP-1s

GLP-1 medications can lead to large weight loss, and losing a lot of weight can mean losing some muscle along with fat. Cleveland Clinic explains that this isn't caused by how the drug works. It's related to the process of losing weight, especially rapid and significant loss. S02 (The doctor quoted on this point is employed by Novo Nordisk, which makes Ozempic and Wegovy.) S02

It matters because muscle is hard to win back. As one Cleveland Clinic doctor puts it, once you start losing muscle mass, it becomes very difficult to gain it back. S03 A joint advisory from four nutrition and obesity medicine groups lists muscle and bone loss among the main challenges of GLP-1 treatment. S07

If you're also noticing changes in your face or skin, our Ozempic face guide explains why those happen.

How much of the weight lost is muscle?

There isn't one number. A 2024 review found a wide spread across trials: in some, lean mass made up 40% to 60% of the weight lost, while in others it was about 15% or less. S05 The authors point out that lean mass isn't only muscle. It also includes organs, bone and water, so a drop in lean mass doesn't mean the same drop in muscle. S05 Based on MRI studies, they suggest the muscle changes on these medicines look largely adaptive, with improved muscle quality. S05

A body-scan substudy of the SURMOUNT-1 trial gives one concrete example. Among 160 adults, people on tirzepatide lost about 21% of their body weight over 72 weeks, including about 34% of their fat mass and about 11% of their lean mass. S06 About 75% of the weight lost was fat and 25% was lean mass, and that split was the same in the placebo group. S06 The study was funded by the drug's maker, Eli Lilly. S06

The review also notes that older age and more severe illness raise the risk of sarcopenia, the loss of muscle mass and strength, which is one reason to plan for muscle health from the start. S05

Strength training is the core

If you take one step for recomposition, make it strength training. Cleveland Clinic calls a mix of strength training and aerobic exercise the way to make sure what you're losing is mostly fat. S03 Its sample recomposition plan uses weight training for the major muscle groups two to three times a week. S01

U.S. guidelines set a floor that works well as a starting point:

  • 150 to 300 minutes a week of moderate aerobic activity, such as brisk walking S04
  • Muscle-strengthening activity, such as lifting weights or push-ups, at least 2 days a week S04

One randomized trial shows why the two belong together. After an initial diet, adults who combined an exercise program with the GLP-1 medicine liraglutide for a year lowered their body-fat percentage by 3.9 points, about twice as much as exercise alone or the medicine alone. S08 Cleveland Clinic adds that with consistent strength training, you may see muscle gains within two to three months. S02

If you're new to lifting or have a health condition, ask your provider what's safe for you and build up gradually.

Protein first, and don't cut too hard

Eating less is part of how GLP-1s work, which makes it harder to get enough protein. Cleveland Clinic suggests working protein into every meal and eating it first, so you don't get full before you have it. S03 Good options include chicken, fish, eggs, Greek yogurt, cottage cheese, beans and lentils. S03

How much protein you need depends on your age, weight, activity and health, so Cleveland Clinic recommends working with a registered dietitian to set the right amount. S02 S03 The joint advisory also lists dietitian counseling as one of the supportive strategies during GLP-1 treatment. S07

Pace matters too. Cleveland Clinic suggests losing weight slowly to help limit muscle loss, and warns against too big a calorie deficit, since you still need energy to support your workouts. S02 S01 Drinking enough water helps with muscle cramps and fatigue after exercise. S02 If you're also noticing hair loss on a GLP-1, eating enough is part of that conversation as well.

How to tell if it's working

During recomposition, the scale can move slowly or even stall while your body changes. Better ways to track progress:

  • Strength: are you lifting a little more, or doing more reps, than a month ago?
  • Everyday signs: Cleveland Clinic counts clothes fitting differently, or being able to walk farther, as a real win. S01
  • Body composition tests: a DEXA scan or bioelectrical impedance can show fat and lean mass separately. S01 The joint advisory recommends assessing muscle strength, function and body composition as part of GLP-1 care. S07

Think long term. Cleveland Clinic describes recomposition as a lifelong practice rather than a destination. S01 For what to expect from your skin as the weight comes off, see our guide to loose skin after weight loss.

Make the habits stick

Recomposition comes down to a few habits kept up for months: regular strength work, protein at every meal, enough water and a steady pace. Olive is a health companion that checks in with you every day, looks at what you eat, and helps you build small habits that actually hold. It doesn't diagnose or treat anything, so questions about your medication or training plan belong with your provider.

Frequently asked questions

Can you build muscle while on a GLP-1?

It's possible, especially if you're new to strength training. Cleveland Clinic says people who train consistently may see muscle improvements within two to three months. Because you're eating less on a GLP-1, keeping protein in every meal matters more than usual.

How much muscle do you lose on a GLP-1?

It varies. A 2024 review found lean mass made up anywhere from about 15% to 60% of the weight lost across different studies. In one tirzepatide trial, about a quarter of the weight lost was lean mass, the same share as in people on placebo. Lean mass also includes water, organs and bone, so it isn't all muscle.

Is muscle loss caused by the medication?

Cleveland Clinic says no. Muscle loss comes from losing weight, especially losing it quickly, not from how the drug works. That's why the same steps that help during any weight loss, like strength training and protein, help here too.

How often should I lift weights on a GLP-1?

U.S. guidelines suggest muscle-strengthening activity at least 2 days a week for adults, and Cleveland Clinic's sample recomposition plan uses two to three strength sessions a week. If you're new to it, ask your provider whether it's right for you and start with lighter work.

How do I know if I'm losing fat or muscle?

The scale can't tell you. Body composition tests such as a DEXA scan or bioelectrical impedance can, and the joint advisory on GLP-1 nutrition recommends checking strength and body composition. Simpler signs count too, like clothes fitting differently or being able to walk farther.

Does body recomposition work if you're older?

Strength work helps at any age, but age affects results, and older adults have a higher risk of losing too much muscle. A 2024 review noted that older age is one reason to think carefully about muscle health on these medicines, so it's worth planning your exercise and protein with your doctor or a dietitian.

Keep the muscle you've built

Losing fat while holding on to muscle takes patience, and the habits that protect muscle are the same ones that help you keep weight off. Olive checks in with you every day and notices what's keeping your body stuck.

If steady habits haven't moved stubborn weight, Olive's Protocol is one option to look into. Prescriptions are only written after a medical consultation with a licensed provider. Not everyone qualifies.
