How to use it
Use the exact name from the package or official page. Record only the claims you can verify and keep unknown information visible. This worksheet organizes shopping information; it does not assess product safety for an individual. Keep your completed copy privately.
What is on each page
- Page 1: instructions and a three-option table for brand, exact name, region, category, source, checked date and stated claim.
- Page 2: package quantity, unit, price, open questions and next action.
How to fill it in
- Write the exact name from the package or the official page, including any suffix or variant, and the region.
- For each claim you care about, copy the wording as the source states it and note where you found it and the date you checked.
- Leave a field blank or write “unknown” when you cannot verify it. A blank is not a negative result.
- On page 2, record package quantity, unit and price in the same unit for every option, then list the questions still open and your next action.
Formats
A two-page PDF designed to print on Letter or A4 and an editable DOCX with the same contents.
